CARETRINITY Solution In Brief

As the prevalence of multimorbidity is increasing globally, placing significant strain on patients, carers, and the healthcare systems, integrated multimorbidity care digital solutions are required for improving services and reducing costs. Service provision is predominantly fragmented and focusing on single disease care, thus not covering the needs of multimorbidity patients. Our consortium proposes the CARETRINITY system, a patient-centric solution valuable from the early detection stage to follow-ups and continuous care. The system enables early identification and risk segregation based on data input from a multitude of sources in a holistic approach. Interdisciplinary collaboration through chat, multimedia, and shared tools, along with improved care coordination particularly tailored for polypharmacy and seamless care transition, are also possible through CARETRINITY, combined with a multi-disciplinary personalised care plan feature. The system promotes empowerment and self-management as well and is, to this end, equipped with a virtual assistant, training material and social interaction services.     

Large-scale implementation of the CARETRINITY solution will benefit procurers not only financially, but also through reducing clerical, increasing interoperability of their technical systems and ameliorating care delivery. Moreover, CARETRINITY will have a positive impact on patients by reducing the number of visits to healthcare professionals and lab facilities, while increasing their quality of life and their trust to the healthcare system for long-term support.

The project CARETRINITY is implemented under the H2020 project “Pre-commercial procurement for integrated CARE solutions addressing the Multimorbidity mATRIX in ageing populations – CAREMATRIX” and is co-financed by the European Union (GA 964370).

The project started in March 2023 and the 1st phase is expected to be completed in July 2023.
